About the Program

This project is funded by the Department of Health and Ageing and is an additional component to the Canning Division’s Infocus Counselling Program.  Psychologists who have completed specialised training are available to provide counselling (contact within 24 hours following receipt of referral) and support for patients who are at risk of suicide or self harm.

  • Eligible patients can access this program over a period of up to 2 months.
  • Intervention can take the form of both face-to-face sessions and phone contact
  • The service is free of charge to patients
  • GPs remain the case manager

Eligibility

The target group for this program are individuals over the age of 16 who:

  • Have been discharged into the care of their GP from Armadale Health Service or Royal Perth Hospital from either the Emergency Department or from a medical ward following an overnight admission after a suicide attempt
  • Have presented to a GP located within the Canning Division’s boundaries after an incident of self-harm
  • Have presented to GPs a GP located within the Canning Division’s boundaries expressing suicidal ideation.

This program will not accept referrals where the patient:

  • Is being managed by a State Government Mental Health Services following discharge.

Please note:

Patients with a long history of suicidal ideation and/or self harm associated with an enduring mental illness are likely to be best treated by the patient's local mental health service, and are not the focus of this program.

This program is not a crisis service and operates Monday to Friday 08:30am - 4:30pm.  Patients in need of immediate emergency response to serious suicidal ideation or self harm should be referred to the Mental Health Emergency Response Line (formerly Psychiatric Emergency Team) on 1300 555 788.


Referrals
GPs referring to the Suicide Intervention Program must:

  1. Complete a Suicide Intervention Program Referral Form and fax to 08 9458 0555 (confidential fax)
  2. Provide the patient a Patient Information Sheet on the program.
  3. Provide the patient with After Hours Crisis Contact List
