What is a Lifestyle Modification Program (LMP)?

The intention of a lifestyle modification program is to help people to modify their risk factors to delay or prevent onset of Type 2 Diabetes. The Healthy Lifestyles Program is an accredited lifestyle modification program incorporating motivation and education to support lifestyle changes and adoption of healthy lifestyle choices. Canning Division also offers RESET YOUR LIFE programs during the evening for eligible referrals For more information regarding Reset your life visit www.resetyourlife.com.au

For patients to be eligible for this FREE program they should be;

  • Age 40-49
  • At "high risk" of diabetes as defined by the AUSDRISK score Enter link to AUSDRISK score, see attached also can we have this link on main canning division web page saying are you at risk of diabetes take the test?
  • Has had diabetes excluded
  • Has had a Health assessment
