What is a Lifestyle Modification Program (LMP)?

The objective of a lifestyle modification program is to support people in modifying their risk factors to delay or prevent the onset of Type 2 Diabetes. The Healthy Lifestyles Program is an accredited LMP incorporating motivation strategies as well as and education to support sustained behavioural modifications and adoption of healthy lifestyle choices. Canning Division also offers RESET YOUR LIFE programs during the evening to accommodate for all eligible referrals.

For patients to be eligible for this FREE program they should fulfill at least one of the following criteria;

  • Aged 40-49
  • At "high risk" of diabetes as defined by the AUSDRISK screening tool (12 points)
  • Has had Diabetes excluded
  • Has had a Health assessment
