This workshop is designed for people who have one or more of a range of chronic conditions
such as diabetes, arthritis or heart disease. In the workshop, participants will learn ways
to self manage their condition in partnership with their health care team. As well as general
information on topics such as physical activity, nutrition, medications, relaxation, symptom
management and communication, the workshop includes practical strategies and skills for coping
with the problems that arise from having a long term health condition.
This workshop is a six week course, with one 2.5 hour session per week. Workshops are held in
a variety of venues and locations to assist clients to access this program. The course is led
by two facilitators in a friendly and supportive environment.

Living Life with Chronic Conditions
This workshop assists people with chronic pain (lasting longer than 3-6 months) to learn ways to self manage their conditions. Participants will gain practical skill on pain management, pacing and learn the Moving Easy Program, an exercise program developed for people wth chronic pain.

Living Life with Chronic Pain
This workshop assists people with Type 2 Diabetes to learn ways to self manage their condition e.g. understanding symptom management, foot care and planning low fat meals. Participants will also learn how to prevent low blood sugar levels and read nutrition labels.
