The HMR (also known as DMMR - Domiciliary Medication Management Review) is a consumer-focused, structured and collaborative health care service provided in the community setting, to optimise quality use of medicines and consumer understanding. It involves the consumer, their general practitioner, their pharmacy, and other relevant members of the health care team.


HMR process

  1. Identify patient - this can be done by GP or other person
  2. GP must see patient - either during normal consultation or at another time
  3. Generate referral
  4. Send referral to patient's preferred community pharmacy - preferably give to patient to take to pharmacy
  5. Pharmacy is responsible to arrange an Accredited Pharmacists to perform the MMR

  6. Accredited Pharmacist completes HMR visit and provides GP with written report
  7. GP sees patient and together, using pharmacist report, they develop and agree to a medication management plan
  8. Copy of plan given to patient and pharmacy

How do you identify patients who need HMRs



According to the Commonwealth Department of Health and Aged Care, below are some common risk factors known to predispose people to medication-related problems and these people are eligible to receive an HMR.

This list is just a guideline as there may be other reasons why a patient may need an HMR:

  • Currently taking 5 or more regular medications
  • Taking more than 12 doses of medication per day
  • Significant changes in medication treatment regimen during the last 3 months
  • Medication with a narrow therapeutic index and/or requireing therapeutic monitoring
  • Symptoms suggestive of an adverse drug reaction
  • Sub-optimal response to treatment with medicines
  • Suspected non-compliance or inability to manage medication-related therapeutic devices
  • Literacy or language difficulties, dexterity problems, impaired sight, confusion / dementia or other cognitive difficulties
  • Attending a number of different doctors, both general practitioners and specialists
  • Recent discharge from a facility/hospital (in the last 4 weeks)
