What is HeartBeat Community Cardiac Care?

HeartBeat WA is a community based cardiac rehabilitation and secondary prevention program that is available to cardiac patients and those at risk of heart disease living in the Perth South East metropolitan area. It is a collaborative program between Canning Division of GP and Royal Perth Hospital and is funded by the Health Department of Western Australia.

GPs can directly refer their patients to the HeartBeat WA Program. For those patients who have been recently hospitalised it provides an effective alternative for low risk patients to the existing hospital based cardiac rehabilitation. All patients need to be registered by their GP by completing a Patient Registration Form. For those patients who plan to join the exercise classes the Medical Profile section of the Registration Form also needs to be completed.

SERVICES FOR PATIENTS:

Exercise Program:

Group exercise classes are held once a week at venues in Armadale and Bentley. Classes are conducted by a Physiotherapist who tailors the program to meet the individual needs of the patient. Patients can progress from the beginners to the advanced exercise class.

Education Program:

Group education and information sessions operate on a 7 week cycle. Sessions are conducted by specialist presenters including nutritionist, pharmacist, physiotherapist and peer support educators. Venues in Armadale and Bentley.

Patient’s carers and family are encouraged to participate in the HeartBeat Program as they play an important role in the health and behaviour change of cardiac patients.

Patient hand held record card:

On commencing the Program patients receive a record card to help them monitor their progress. This provides space to document management goals for risk factors, medical review dates, test results and record participation in education and exercise.

SERVICES FOR GPs

  • Community contacts for cardiac care (for example, physiotherapy, nutrition & weight loss, counselling, vocational, psychosocial and smoking)
  • Care plan information and support
  • Reminders for follow-up to patients and GPs after three and nine-months of program participation
  • Information on activities for cardiac health in the local community
