The Healthy Lifestyle program is for patients with either type 2 Diabetes,
Multiple risk factors for Coronary Heart Disease. It includes initial
assessment of patients and a group education / exercise program free to
patients. The program encourages liaison with GP's to maintain good progress.
The healthy Lifestyle program is also an accredited Lifestyle Modification Program.
What is a Lifestyle Modification Program?

The program includes free access for patients to appropriate members of the allied
health team. Patients referred to the program will be assessed, educated and
supported by:
- Accredited Practicing Dietician
- Credentialed Diabetes Educator
- Exercise Physiologist
The program targets people with any of the following:
- Type 2 Diabetes diagnosed < 5 years
- Type 2 Diabetes with microalbuminuria
- Multiple risk factors for CHD
- IFG and IGT
The program is provided free by the division and includes an initial assessment
of the patient to determine which services / programs maybe required. If
suitable patients are then referred into an 8 week group education / exercise
program to provide structured education on lifestyle changes and chronic disease
management.
In addition patients are provided with additional ongoing support and guidance
for up to 12 months from referral by the healty lifestyle team.

- Access to group exercise / education program
- Individual follow up
- Guidance on access to other appropriate services
Practical support to GP practices including assistance to develop GPMP and TCA
for patients when appropriate. Program officers will also support practices in ongoing
chronic disease management and use of recall system.
- GPMP and TCA are completed for you to review with the patients and claim
- Progress reports to GP's
- Patient referral to other services where identified as necessary
for ongoing support and education
- You may be eligible to claim a 713 if the patient scores>15
on the
