Hospital GP Liaison Princess Margaret Hospital - Dr Maree Creighton Royal Perth Hospital - Dr Jacquie Garton-Smith King Edward Memorial Hospital - Dr Vicki Westoby Sir Charles Gairdner Hospital - Dr David Oldham Fremantle Hospital - Dr Debbie McKay Nurse led Diabetes Clinic SCGH A new nurse-led clinic is starting in Diabetes Out Patient Department at SCGH for cardio-vascular risk reduction in patients with type 2 diabetes. Patients will have initial assessment and management plan by Dr Joey Kaye (Diabetes Specialist) and will then be offered additional follow appointments with Alison Jones who is a Diabetes Educator. Alison will send a letter to the referring GP summarising the patient visit after each appointment.This is an additional service.
