What is the COPD Community Linkage Service?

The COPD Community Linkage Service provides integrated community based management of COPD. It aims to strengthen community based care for people with COPD and reduce unplanned hospital admissions. The service model has been developed with an evidence based approach and emphasises early community intervention, GP led care coordination and prevention.

Services for Patients

This multidisciplinary service provides fully integrated care, including:
  1. Respiratory Nurses providing client surveillance, case management support and GP/practice nurse up skilling and liaison;


  2. Respiratory Physiotherapist providing patient surveillance, education, prescription of home and group based rehabilitation programs;


  3. Respiratory Physician providing specialist expertise, support for the clinical staff and team leadership;


  4. Multidisciplinary COPD Clinics providing specialist input from the Service Respiratory Physician, Physiotherapist and Nurses;


  5. Integration of Community Pulmonary Rehabilitation services;


  6. Peer support for patients with COPD
