The Aged Care Access Initiative delivered by Canning Division of General Practice aims to improve access to allied health services for permanent residents of residential aged care facilities in the region.
Canning Division conducts this initiative on behalf of the Commonwealth and provides Speech Pathology and Dietetic services to residents of these facilities where these services are not funded under the Aged Care Act 1997 or through any Medicare or other Government funded programs. Services delivered to residents do not incur any cost to them or the facility.
All residential aged care facilities within the Division’s boundaries are eligible to request services on the appropriate Referral Form available below. It is also a requirement that facilities accessing these allied health services report back to the Division on the services provided and a Service Record form is available below for this purpose.
In order for us to evaluate the program, aged care facilities utilising these services are required to complete a brief annual survey which is sent to them in April each year.

of frail aged or people with disabilities. The Crisis Care Service is for those unforeseen
incidents that result in the carer being unable to continue in their caring role. The service
